- The most recent Form DS-2019, Certificate of Eligibility for Exchange Visitor (J-1) Status, issued by the exchange visitor program in which you plan to participate.
Be sure the personal information on your Form I-20/Form DS-2019 is correct before proceeding to pay the I-901 Fee - particularly your name, birth date and your school code/program code. If this information is not correct, do not continue to pay the SEVIS I-901 Fee. Contact your designated school official or program sponsor for a corrected I-20 or DS-2019.
